Rady Children's Physician Management Services (RCPMS) offers exciting opportunities for qualified candidates to join our rapidly growing organization. RCPMS supports primary care pediatricians with 32 office locations throughout San Diego and Southern Riverside Counties.
Under the supervision of an Associate Director and in collaboration with the CPCMG Director of Population Health, the Population Health Clinical Manager has oversight of Population Health programs including but limited to patient care coordination, data collection and resource management. The Population Health Clinical Manger works in partnership with the Population Health Manager on the broader scope of Population Health to improve outcomes of specific patient groups based on identified clinical risk, SDOH and community factors. The Population Health Clinical Manager provides clinical expertise and oversight of the development of Population Health initiatives, studies, and pilots and will perform due diligence to identify specific patient populations through vetted data and reporting requirements. The Population Health Clinical Manager directly supervises RN Care Coordinators and Specialists and partners with various RCPMS departments, CPCMG leadership and applicable embedded external resources on population health initiatives.
Responsibilities:
- Works collaboratively with Associate Director and CPCMG Population Health Director to generate ideas for Population Health and Care Team supported workflows.
- Develops goals and objectives for the Population Health department and ensures implementation of operating plans.
- Develop and implement policies and procedures that have direct impact for the day-to-day operations.
- Monitors productivity and workflow, achieves budget targets, and ensures operational efficiency and quality of services
- Provides routine and ad hoc reporting for various Population Health initiatives and pilots.
- Develop and enforce policies and procedures for patient-centered care management encompassing various Population Health functions.
- Works closely with various RCPMS departments including Quality and Epic services to develop workflows including procedures and protocols.
- Coordinates with Ops and CPCMG leadership on various population health initiatives through Center of Excellence programs.
- Collaborate with other CPCMG ancillary programs (i.e., Mental Health, Newborn, Lactation, etc.) to eliminate redundancy in care coordination.
- Act as a liaison between community partners such as CBOs, RCHSD, payors and CPMG
- Oversees, maintains, and ensures proper tracking of patient outreach and appropriate documentation of patient encounters.
- Participates in various committee/task force work.
- Travel and work between various CPCMG locations.
Requirements:
- Graduate of NLN accredited school of nursing.
- Bachelors of Science degree in Nursing highly desired.
- Current RN license in the State of California
- CA Drivers License and insurance
- 2 years supervisory experience, such as charge nurse, supervisor, lead or manager
- Has experience with one or more of the following areas: case management, care coordination, population health, public health, nurse educator, quality management or utilization management
